Volunteer Leaders of the CDSMP

The CDSMP is designed to give people living with chronic conditions the skills and tools to help themselves manage the everyday challenges of living with their chronic conditions. The course, Living a Healthy Life with Chronic Conditions, is co-facilitated by pairs of volunteer leaders, many whom have chronic health conditions themselves are successfully using the strategies they have learned through the six week course.

Volunteer Leaders have the distinct pleasure of observing people affected by chronic conditions successfully manage their conditions using the skills and tools shared with them during the six week course.
